PAIN MANAGEMENT PATIENT (SEEKING MEDICATION MANAGEMENT THERAPY) REQUIREMENTS

All patients should be twenty-five years of age or older; some exceptions may be made for particular cases at the doctors’ discretion.

We require that our patients have a verifiable MRI and/or CT scan that was performed within the past two years;

If you need to an MRI or CT scan done our physician will gladly order one for you.

We refer our patients to two separate Imaging centers that both perform same day services. One office has a low self-pay price of $280 and the other is able to accommodate our patients needing to use insurance.

All patients must also have proof of prescription history i.e. pharmacy records, medication bottles, medical records, etc. This is needed so our physician knows what and how to prescribe the appropriate medication for your condition. Patients who cannot provide any medication history will be treated at the discretion of our responsible physician as new pain management patients.

If the doctor feels that you have been inappropriately prescribed and/or over-prescribed medications she will adjust your regiment accordingly keeping your physical and mental well being a priority.

All patients also go through an initial and periodical drug screening and all females are given pregnancy tests at each visit to insure their safety as well as the safety of their child.

NEW PATIENT RECOMMENDATIONS

We do prefer appointments but if you are not able to pinpoint a time that you will be arriving at our office we ask that you print and fill out the provided paperwork to shorten the wait time at our office.

Also, if you are a new patient we prefer that you fax or email us a copy of your MRI/CT scan and all medical and pharmacy records at least 24 hours prior to your arrival so we can facilitate your visit.

If you do not have the above records on hand you can print, fill out, and return our medical records request form for the appropriate offices i.e. MRI/CT scan imaging center, pharmacy, previous physicians office, etc.

Our staff will contact all of the necessary practices to acquire your records before you arrive in order to make your visit as timely as possible.

This will save you precious time which you may otherwise sacrifice sitting and waiting in our office as we do not allow patients to be treated until ALL medical records are obtained and verified.
